- Provide personal care assistance to elderly with disabilities, dementia, or other special needs.
- Assist with activities of daily living (ADLs) such as bathing, dressing, grooming, and toileting.
- Support individuals in maintaining a clean and safe living environment.
- Help with meal preparation and feeding if necessary.
- Assist with medication reminders under the supervision of a nurse or healthcare professional.
- Provide companionship and emotional support to individuals.
- Accompany individuals to medical appointments, social outings, and other activities as needed.
- Follow care plans and instructions provided by healthcare professionals or case managers.
- Document and report any changes in the individual's condition or behavior.
Requirements:
- Previous experience working with elderly with disabilities, dementia, or other special needs is preferred.
- Knowledge of social work principles and practices is a plus.
- Ability to lift and transfer individuals who may have limited mobility or require assistance.
- Familiarity with hospice care systems and protocols is beneficial.
- Excellent communication skills and the ability to work well with others in a team environment.
- Compassionate and patient demeanor when working with individuals who may have challenging behaviors or communication difficulties.
- Willingness to undergo any necessary training or certifications required for the position (e.
g.
, CPR/First Aid).
- Ability to follow instructions and adhere to established care plans.
- Flexibility in scheduling to accommodate the needs of the individuals being cared for.
